If Grid/3D parameter is disabled, the cloud will be projected to ground. What is the frame_id in the planner_cloud? The map frame would not be used for planner_cloud if rtabmap is not used. How did you configure the obstacles_detection nodelet?
If /rtabmap/cloud_ground is empty, it depends what the camera is seeing and the Grid/ parameters set. If you have a database, it will be easier to see why.
